Members of Boston PCN 

Boston PCN is a network of six practices that collaborate to improve patient health.

To support them in delivering additional services there is a PCN Management Team.

This is the team:

Dr Zubair Alam - Clinical Director

I qualified as a GP nearly 15 years ago and have been a GP partner at Swineshead Medical Group for over 13 years. I am a qualified GP trainer for registrars and other auxiliary staff and have a keen understanding of how different role compliments each other to create a team that can provide the best care to patients, not only clinically but within the wider health and social care system. I have a special interest in diabetes and dermatology and have completed diplomas in both. During this training I gained and understanding of the importance of integrated teams and how complimentary relationships between Primary and Secondary care, can reduce pressure on hospitals and provide local care for our patients. 

I feel this post will allow me to further enhance the healthcare services provided locally. It provides an opportunity to bridge the gap between local health provider and the hospitals and implement the change necessary to match the demands and expectations of the patients.

Dr Divya Kuttappan - Deputy Clinical Director

Dr Kuttappan obtained her basic Medical Degree from India in 2000 and completed GP training in Boston in 2016. Since then she has been working as salaried GP in Lincolnshire. She has done a Fellowship in Diabetes in 2018 and is currently a GP Educator. She has special interest in population health management and Lifestyle Medicine. She was actively involved in setting up of the Boston Junior Parkrun and is one of the Event Directors.

 

Jodie Lindley - Director of Strategy and Partnerships 

I qualified as a mental health nurse in 2001 and have worked in secondary mental health services for most of the years since; as both a community nurse and as a manager across the East Mental Health Teams. During this time, I worked with primary care to develop and embed the community mental health transformation work, and during this time enjoyed working with Boston PCN which led to my current role as the Director of Strategy and Partnerships, which I love. I am truly passionate about supporting patients to be able to access the right healthcare when its needed and for them to have smooth transitions into services. Part of my role is to establish good working relationships with our stakeholders services such as Lincolnshire Community Health Services and United Lincolnshire Health Services, along with our community partnerships and voluntary sector organisations.

I am committed to ensuring that we address health inequalities and having fair access for everyone in our population and ensuring that we have the right skills and experiences in our practices to support the needs of our patients.

I am also passionate about Women’s health and have a background in Perinatal Mental Health and Health visiting.

 

Emma Woods - PCN Manager

After graduating from the University of Leeds in 1998 with a degree in social policy I began my public sector career in local government Best Value, supporting various departments in monitoring and improving the quality of the services they provided. From there I moved into the NHS and worked for a decade as part of the clinical governance and risk team in East Lincolnshire PCT, based initially in the South Holland area initially and then moving to Lincolnshire Teaching PCT following reorganisation. I worked with GP practices in the locality on the clinical governance agenda as well as supporting the implementation of the nGMS contract, QOF and the Investing in Primary Care Framework. I also worked with community based health teams on implementing their clinical governance strategies as well as developing audit tools for the Essence of Care benchmarks.

I then took a few years out to run the office of my husband’s family farming business while our children were young and re-joined the public sector in 2016 and worked as the school business manager for a federation of two local primary schools, just outside Boston.

In October 2024 I took up the post of Boston PCN Manager and I am looking forward to working with my colleagues across the PCN to deliver effective primary healthcare services to our patients.

Ben Sandall - Digital and Transformational lead 

I was awarded my degree in business and enterprise management in 2017. Since then I have held numerous project manager roles within the NHS working across a total of 42 general practices. I have procured care navigation training for over 400 members of staff, set up home visiting services covering a population of 210,00 people, worked with countless teams to optimise digital opportunities to increase efficiency/patient safety.  I have also worked in the finance sector as a day trader, having a sound understanding of risk management combined with a winning strategy to generate returns for clients. As a local resident I am passionate and the progressing the digital maturity of our PCN.
