Boston Health and Wellbeing Team

We are here to support the health and wellbeing of Boston PCN patients, to help individuals make positive and lasting changes to their health with a personalised approach.

The Boston Primary Care Network is 6 GP practices: Liqourpond Surgery, Kirton Medical Centre, Greyfriars Surgery, Swineshead Medical Group, Parkside Medical Centre and The Sidings Medical Practice.

The team can support with various aspects of health, including sleep, wellbeing, physical activity, healthy eating, and lifestyle choices. We aim to encourage you every step of the way, sharing tools and techniques to last a lifetime.

How long will the support last?

We are all unique and different. Our sessions are tailored to you and your needs. We will take the time to work with you over some sessions, book weekly/fortnightly appointments as appropriate. We will agree between us several sessions which we will form part of our overall target for you.

We have a team of incredible staff who are working hard to deliver our new service. Our team consists of health and wellbeing coaches, an occupational therapist and social prescribing link workers. with the hope of expanding the team to broaden our skill set.

Health and Wellbeing Coaches

Our health and wellbeing coaches are qualified individuals who aim to work with you to identify how we can help and motivate you to reach your personal goals. We do this by spending time with you to identify what matters to you most, helping you set realistic goals and putting these plans in motion. Sometimes, people need more time and support. this is what we aim to achieve, helping you to make lasting improvements to your health and wellbeing.

Occupational Therapist

 Occupational therapists take on a ‘whole person’ approach, working alongside you to identify and achieve your goals, enabling you to live a full and satisfying life. We can work with you to improve or maintain your independence and participation in everyday activities. This could include changes or adaptations to your routines, environment or altering your approach to tasks. We are focused on what matters to you, not what is the matter with you.

Social Prescribing Link Worker 

Our social prescribing link workers are all about connecting you to the community resources. We do this by helping to enhance your health and wellbeing by supporting you to increase social interaction, build confidence, self-esteem, enhance self-care and build knowledge of local resources. Short term one to one support and advice aims to reduce social isolation and connect you with your local services.
